You wrong. formative construct => indocators to construct. You need questionnaire items if unobseverd variable.
For evaluation outer model construct formative, you look significant weight, assess content validity and for addition, your check external reliability.
For R-squares and significant weight, you run PLS algorithm and bootstrap procedure.
