Literature on how to develop questions to measure SEM?

Literature on how to develop questions to measure SEM?

Dear all,

Usually when measuring indicators with questions in a survey researches tend to use questions/scales from existing publications.
In my case there is no similar measurement in literature. Thus, I have to develop questions (and scales) by myself. There are a lot of books and articles on how to define a structural equation model, but there is only little information how to really measure it. Only in rare occations, research papers include the survey questionnaire in the annex. So often the model is described in detail in the paper, but lacks information about the underlying questionnaire.
The only thing I came across regarding questions and scales was a Marketing Scale Handbook. But this is suitable only for Marketing related research.

Can anyone recommend good literature/websites on how to develop questions to measure the model and what scales to use?
